  • General review

This is an AI podcast as evidenced by mispronunciation of words like meningocoele so understand it can’t be perfect but should be quite good. The chronic subdural episode has some inaccurate information and needs to be revised. It would be useful to address rates of complications and the literature behind this. Please also note that the volume is variable and usually low. Overall a good start but lots to work on.

Craniopharyngiomas
Q: If you see a solid, well-circumscribed craniopharyngioma with clear papillary architecture on histology, and it shows strong cytoplasmic BRAFV600E positivity on IHC, which subtype are you almost certainly dealing with?
That pattern points directly to papillary craniopharyngioma, PCP.
Craniopharyngiomas
Q: What specific histological feature, often described as ghost cells or wet keratin, is uniquely characteristic of adamantinomatous craniopharyngioma and is never found in papillary types?
That unique histological feature is wet keratin or ghost cells.
Craniopharyngiomas
Q: Which specific genetic mutation is typically associated with adamantinomatous craniopharyngioma, and which key cellular pathway does it activate?
The mutation is CTNNB1 encoding beta-catenin, and it activates the Wnt pathway.
Extradural haematomas
Q: Why might the presentation of EDH in pediatric patients occasionally be subacute, differing from the more typically acute presentation seen in adults?
In pediatric patients, EDHs can sometimes present subacutely because the bleed may be from lower pressure venous sources, meningeal or diploic veins, rather than arterial; also, the dura is more strongly adherent to the pediatric skull.
Extradural haematomas
Q: On an acute CT scan of an EDH, what does seeing a low density swirl within the density of the hematoma specifically indicate?
It indicates active bleeding, with fresh, unclotted blood extravasating from the torn vessel into an already organizing clot.
