Talking Points

Recent interactions between the hosts and their guests.

Pediheart Podcast #364: ECMO Prior To Single Ventricle Palliation - Outcomes
Q: Do you feel that the impact of this work is mostly in providing prognostic data to families, or do you think that there may be some medical management impact from your work as well?
There are two main takeaways: prognostic clarity for families and clinical management implications, such as identifying and supporting end organ function early to avoid fluid overload states.
Pediheart Podcast #364: ECMO Prior To Single Ventricle Palliation - Outcomes
Q: Do you think renal replacement therapy is a surrogate for worse general body perfusion?
Yes, renal replacement therapy likely reflects significant global hypoperfusion and multi-organ compromise rather than being solely an isolated renal issue.
Pediheart Podcast #364: ECMO Prior To Single Ventricle Palliation - Outcomes
Q: Why do you think that single ventricle ECMO patient survival has not really substantially improved?
Survival has plateaued because the physiology is very unforgiving. By the time patients require ECMO, they often have significant multi-organ injury that ECMO can't always reverse.
Pediheart Podcast #364: ECMO Prior To Single Ventricle Palliation - Outcomes
Q: What do you think are the three or four most important observations of your work?
The overall survival to hospital discharge remains low at about 42%. The duration of ECMO support is a strong predictor of outcomes. Infants who did not survive had substantially longer ECMO runs than those who did, and the need for renal replacement therapy is associated with markedly higher mortality.
Pediheart Podcast #365: Global Inequity In Pediatric CHD Care - Sobering Facts And A Way Forward
Q: Why does the development of a World Health Assembly resolution help improve care in low or middle income countries?
It can drive action towards taking care of congenital heart disease by providing a framework for countries to strengthen their healthcare systems.
